Administrative Access
Information in your patient record is confidential.Without your permission, it will not be given to anyone, except those involved in your treatment and your own GP.
Following discharge, if you wish/need to receive a copy of any part of your patient record you may do so by writing to:

Please give the following information, to enable your record to be identified:
your name
date of birth
address- current and previous if applicable
contact telephone number
your patient record number – if known.
Also, be specific as to what attendance/admission records you require.
When the relevant records are copied , you will be required to produce Photo ID such as your passport or drivers licence. This is to protect the confidentiality of your records.

An administrative fee of €25 is charged per request of health care record. X-Rays are charged at a fee of €10.00 per film.
Request by Solicitor

Under no circumstances will a copy of a patient’s records be released to a solicitor, without the patient’s consent. The fees payable by solicitors in respect of copies of patient records are as follows:
- A & E Notes = €50
- Patient Chart = €50 per volume
- X-Ray = €10.00 per film

The Freedom of Information Act 1997, was designed to allow individuals access to information held by Public Bodies which is not routinely available.
To make a request under the Freedom of Information Act, your request must be in writing and addressed to:
